Maternity and Antenatal Self Referral services

If women wish to self-refer for care they can now use the online self-referral form which can be found on our website here - completed online referrals will be sent directly to the booking administration team.  If women are not comfortable using the online referral form they can present at the community midwives centre or midwives house where a referral form can be completed manually. If women have queries and cannot get through to their midwifery team then they should call 0203 299 6010.  This number can also be used for new bookings.

For enquiries regarding test results, there is now a  test results line for women and GPs  to be contacted on– 0203 299 1311
